✔️ How does Depram 25 work?

Depram 25 effectively reduces the symptoms of depression and it works by increasing the concentration of neurotransmitters called norepinephrine and serotonin as this substance is found to be low in patients with depression. Through its anticholinergic effect (blocking acetylcholine), imipramine controls involuntary muscle movements, thereby it provides relief from nocturnal enuresis or bedwetting in children.

✔️ Indication of Depram 25

Imipramine (Depram 25) is used for the treatment of symptoms of depressive illness. It also provides relief of nocturnal enuresis (bed wetting) in children. 

✔️ Directions for Use: Depram 25

Swallow Depram 25 as a whole with a glass of water, do not chew or crush it.

✔️ Dosage & Administration of Depram 25

For Depression:

• Adults: Start with up to 75 mg daily in divided doses, gradually increasing to 150-200 mg/day (up to 300 mg for hospitalized patients). Up to 150 mg can be given as a single bedtime dose.
• Elderly: Start with 10 mg daily, gradually increasing to 30-50 mg/day.
• Children: Not recommended for depression treatment.

For Nocturnal Enuresis:

• Children (7-8 years): 25 mg at bedtime.
• Children (8-11 years): 25-50 mg at bedtime.
• Children (over 11 years): 50-75 mg at bedtime.
  Treatment duration, including gradual withdrawal, is 3 months, with a full physical examination required before repeating the course.

Children are susceptible to imipramine overdose, which can be serious or fatal. Close monitoring is essential for any suspected overdose.

✔️ Interaction

• Increased effects: Methylphenidate may enhance imipramine’s effects.
• Avoid alcohol: Depram may increase alcohol's depressant action.
• Severe interactions with MAO inhibitors: Imipramine should not be taken with MAO inhibitors (e.g., phenelzine, tranylcypromine). A gap of 14 days is required between discontinuing one and starting the other to prevent life-threatening reactions like hypertensive crises, seizures, and coma.
• Additive anticholinergic effects: When taken with antihistamines, benztropine, and biperiden.

• Concurrent or recent (within 14 days) use of MAO inhibitors
• Severe liver or kidney impairment
• History of blood disorders
• Hypersensitivity to tricyclic antidepressants or dibenzazepine derivatives
• Acute recovery phase following a heart attack
• Glaucoma, convulsive disorders, or urinary retention

✔️ Pregnancy & Lactation

• Pregnancy: Category D. Known risks to the fetus.
• Lactation: Likely excreted in breast milk. Use with caution.

✔️ Precautions & Warnings

• Use cautiously in elderly patients, particularly those with benign prostatic hypertrophy, glaucoma, or cardiovascular disease.
• Avoid combining with alcohol or other sedatives.
• Monitor for signs of suicidal thoughts, especially during dose initiation or adjustment.
• Risk of manic episodes or exacerbation of pre-existing psychotic states.
